Welcome! If you are a patient, please sign in using the user-id and password you were assigned. If you have forgotten, or not received a user ID, please call 586-203-2020 to obtain this. Signing in will allow you at your leisure to complete necessary forms prior to your visit. Heres what to do:
--Login to the "Client Login" section and you will be taken to the HOME screen where you will see a "MY TO DO LIST". Please click on each link under the heading "MESSAGE" and then read and complete each form I have initially placed in your "MY TO DO LIST". Once you have read the required form scroll down to the bottom of the page and enter your password and click "SIGN AND COMPLETE DOCUMENT."
The information you submit is secured and no one else has access to your clinical information as it is password protected .Once logged in, you will have the option of changing your password by clicking on "change password" on the left top portion of the screen. In order to get back to the main page to continue with the "To Do List", click the" back" button at the bottom of each form. For client's under the age of 18, a parent or guardian will need to fill out each form for the client and electronically sign each form.
